My name is Weldon R. Grisham. I am an attorney. I have been licensed to practice law by the Texas Supreme Court since 1979.

The information you put on the document linked below will be seen only by me.  Therefore, mark on the sheet, use the back of it, color on it, spill coffee on it -  but please fill it out.  If you do not know the answer, put a question mark (?).  Or if it does not apply, put "N/A" for non-applicable.  The dollar amounts may be close guesses or estimates.  If you brought a LIST of your creditors, you do not have to recopy them on the Information Sheet.  But please answer the other questions.  If you brought your invoices (bills) from your creditors, please list each in the following pages with the dollar ($) amount owed to each creditor.
